Book if: Your card is expired or expires in the next 60 days.
Includes: Patients previously certified by other doctors.

Medical Forms
Book if: You need "reasonable accommodation" or other forms for work or school filled out by a physician you have previously been certified with.

Diagnosis/Initial Certification
Book if: You have never had a PA medical card before and need a medical diagnosis for a qualifying condition. (Applies only to Anxiety, PTSD, and Chronic Pain)
Requirements: Valid PA State ID/DL and potentially qualifying symptoms.
